Step 1 Talk to Anna
Talk to Anna (outside around (7, 52) in the morning and during the day, in her house in the evening and at night) and choose Talk about the Bell That Wasn't There.
Step 2 Search the shrine
Enter the Havenreach Shrine (door at (29, 10) in Havenreach) while you have Spectral Sight. A hidden mark appears at (3, 6). Pick it up to get the Pale Chime Fragment.
Step 3 Tell Anna
Talk to Anna with the fragment.
Step 4 Show Isak
Talk to Isak around (54, 16) in the Overworld with the fragment in your backpack.
Step 5 Return to Anna
Give the fragment to Anna to complete the quest.

The mark only appears during this quest, only with Spectral Sight, and only while you do not already carry the fragment.
